Support at Home
At Onward Health, we provide occupational therapy services under the Australian Government’s Support at Home funding model. Our services are designed to help older adults remain safe, independent and confident in their own homes for as long as possible.
We understand that changes in mobility, cognition, strength and balance can impact everyday activities. Our approach is practical, respectful and delivered within the home environment to ensure strategies are meaningful and effective.
What We Do
Support at Home is the Australian Government’s aged care funding program designed to help older Australians continue living independently at home.
If you are eligible for government-funded home care services, occupational therapy may be included as part of your approved supports. We work collaboratively with care coordinators, providers and family members to ensure services align with your care plan and goals.

Functional Assessment & Independence Planning
We assess your ability to complete everyday activities such as dressing, bathing, cooking and moving safely around the home. We develop tailored strategies to maintain independence and reduce risk.
How We Can Support You
Assistive Technology & Equipment Prescription
We recommend and arrange equipment such as mobility aids, bathroom supports, seating solutions and pressure care devices to enhance safety and comfort.
Falls Prevention & Safety Planning
We assess environmental risks and develop practical strategies to reduce falls and improve safety at home.
Carer Education & Support
We work with family members and informal carers to provide practical strategies that reduce strain and promote safe support within the home.
Cognitive & Upper Limb Rehabilitation
We provide cognitive retraining and upper limb rehabilitation to support memory, attention, coordination, and functional use of the arms and hands following stroke, neurological conditions, or injury.
Home Safety & Modifications
We assess and recommend modifications such as grab rails, ramps, step adjustments and bathroom accessibility supports to improve safety and function.
